Independent commission to conduct an independent, thorough, objective incident review of the May 31, 2019, tragedy and make recommendations regarding improvements that can be made in the Commonwealth's laws, polices, procedures, systems, and institutions, as well as those of other governmental agencies and private providers, to minimize the risk of a tragedy of this nature from ever occurring again in the Commonwealth.
